Step 1 : Ensure That Your Chosen Agency Works In Your Geographical Area.
Renaissance Home Care works in Allegheny, Beaver, and Washington counties.
Step 2 : Contact Your Assigned Service Coordinator At Your Current Home Care Agency.
Your Coordinator will be able to help you with the whole process, including answering any questions that you may have.
Step 3 : Complete Your Health Plan’s Paperwork To Confirm The Change.
The paperwork will include information about your new agency and may require you to authorize the switch. Return your paperwork to the Service Coordinator or health plan as instructed. Be sure that your Service Coordinator has everything they need to complete your application.
Step 4 : Discuss Your Needs With Your New Service Coordinator At Renaissance Home Care.
Once you’ve switched, we’ll schedule an assessment appointment with one of our nurses so that they can get a better idea of what services are the best fit for you.
